Release of Liability

I, on behalf of myself, my child, release, discharge, and hold harmless United Sports FC, its administrators, directors, coaches, volunteers, and any affiliated entities, from any and all liability, claims, demands, actions, or causes of action arising out of or related to my child's participation in United Sports soccer activities, including but not limited to, any injuries, damages, or losses incurred during practices, games, or other related activities.

Assumption of Risk

I understand that soccer is a physical sport that involves inherent risks, including but not limited to, collisions, falls, and contact with other players and equipment. I acknowledge that my child's participation in the soccer program carries the risk of injury and I assume all such risks on behalf of my child.

Photographic and Video Release

I grant United Sports FC the right to use, reproduce, and/or publish photographs and videos taken during info night for promotional or educational purposes without compensation or further authorization.
